Our service is:

• For adults who need therapy after stroke
• For people with and without a medical card

How do I access the service?

• Get a referral form (referral form)
• Give it to your doctor (GP or Consultant)
• The doctor fills in the form with medical test results e.g. brain scans
• The doctor sends the form to us
• We contact you to arrange a time to meet
• We discuss your concerns with you and your family
• We discuss what therapy you need with you
• We arrange the therapy appointments
• We have a limited transport service
